Run-and-gun in the established Metal Slug shape: you advance right, shoot everything, and free prisoners of war who hand over better weapons for the trouble. Marco, Tarma, Eri and Fio are fighting the Rebel Army again, though by the closing mission the enemy has turned Martian. There are five missions, a pistol with unlimited ammunition, grenades on a second button, and enemy fire that kills you in a single hit.
The addition this time is branching. The first four missions fork, sometimes more than once, into routes of very different length and difficulty — one path may drop you underwater, another underground among mummies — while the fifth mission runs as a single linear stretch. The vehicle roster expands too, adding the two-armed Slugnoid, a drilling machine, an ostrich, an elephant and the Rebel Army's LV Armor walking suit, and status effects like the zombie transformation rewrite your attacks rather than merely punishing you.
This is where the series peaks in sheer density of ideas: transformations that swap your entire moveset, routes worth replaying to see what you missed, and a closing mission that abandons branching altogether for one long linear run ending aboard the alien mothership Rugname against Rootmars. The hand-drawn animation carries the same detail throughout, from individual soldiers' panic to each vehicle's damage states.

You cannot crouch or jump properly and you lose access to weapon upgrades and vehicles, but the grenade button charges a blood spray that hits nearly the whole screen, and you become immune to almost all ordinary Rebel attacks. Picking up a first-aid kit turns you back into a human.
There is no correct answer; the branches differ in length, vehicles and enemy types, and all of them rejoin for the same boss fight. Shorter paths finish faster, while longer routes hand out more prisoners and weapon drops. Seeing both is the point of the design.
